Accident Reports

Forms and Documents

Purchase a copy of a traffic crash report prepared by Chesterfield Township officers online through LexisNexis, 24 hours a day. Records staff can help if you don't have a computer.

Traffic crash reports (UD-10) prepared by Chesterfield Township Police Department officers are sold online through LexisNexis, available 24 hours a day, 7 days a week. Involved parties, insurance companies and attorneys can search for a report, pay for it and download it.

When searching the online portal by report number, enter numbers only. Do not include spaces or dashes. A report appears online once the officer has completed it and it has been reviewed, so allow a few days after the crash.

If you don't have a computer or internet access, come to the Records window during public hours and staff will help you get your report. Other requests for crash reports are handled as FOIA requests.

What You Will Need

  • The date, approximate time and location of the crash.
  • The name of at least one party involved in the crash.
  • The crash report number, if you received one at the scene.
  • For online searches, the report number entered as numbers only, with no spaces or dashes.

How to Submit
