All persons requesting a PBT as of November 1st, 2014 will be charged $70.00 for a PBT Punch Card that will be good for seven (7) tests. This charge is in effect whether you require one (1) test or seven (7) tests. You will be required to present the card at the time of the test and it will be punched for each test given.
It is the responsibility of the purchaser to maintain the card that is issued. The Chesterfield Township Police Department will not replace lost, misplaced, stolen, or mutilated cards. If the punch card is lost, misplaced, stolen, or mutilated the requester will need to purchase another punch card before a test is given.
The PBT Punch Cards will be issued and tests given during the regular business hours of the Chesterfield Township Police Department Records Section: Mon/Wed/Fri 8:00 am – 4:30 pm or Tues/Thurs 1:30pm – 4:30 pm
